FWD Insurance special offer (60K medical + 0% co pay + indemnity)
14 months ($393.50) / 26 months ($557.28)

Cost = Agency Service Fee + Additional Services Charge + Insurance

Example: Employer renew for 2 years contract
Cost = $200 + $120 + $605.80 = $925.80

Processing:

  1. Employer need to discuss with MDW with next contract salary and off day agreement.
  2. Employer choose 14 month or 26-month insurance package
  3. Authorize agent to do renew on behalf of employer
  4. Using agent contract to sign together with MDW
